The procedure

Dr Manon Belhoste performs two formats of gynaecomastia surgery, depending on complexity. The excess is removed by liposuction, by direct removal of the gland, or by both at once. When the skin is overstretched, a piece of skin is removed and the areola may be raised. The choice is made during the consultation, based on the proportion of fat and gland, the condition of the skin and the position of the areola.

Liposuction alone

Removes the fat and gland by suction.

Before operating, a reversible cause is sought: recent gynaecomastia may regress after a medicine or substance under medical supervision is stopped or adjusted, or after an underlying condition is treated. Guidelines reserve surgery for forms that persist or whose impact justifies definitive treatment. The limits: asymmetry, a hollow below the areola, residual excess, irregularity or recurrence remain possible.

Before gynaecomastia surgery, Dr Manon Belhoste and the anaesthetist give instructions on fasting and medication, which must be followed strictly. Stopping tobacco and nicotine completely is required: they damage microcirculation and increase the risk of delayed healing, scar reopening and necrosis. All treatments and supplements must be reported, especially anticoagulants, antiplatelet drugs, anti-inflammatory drugs, hormone treatments, anabolic steroids and herbal remedies. None should be stopped on your own initiative. Report any fever, cough, infection or recent skin lesion, and bring the prescribed tests on the day of the procedure.

After gynaecomastia surgery, pain, chest tightness, bruising and swelling are common in the first few days. Dr Manon Belhoste gives you instructions on dressings, showering, scar care and sun exposure at discharge. Scars often stay red, firm and visible for the first few months, then fade. Scar maturation takes 12 to 18 months, sometimes longer. Gentle walking is recommended from the first few days. The chest, areola or nipple are often less sensitive at first. Sensitivity usually returns gradually.

Gynaecomastia is a benign increase in breast volume in men, caused by proliferation of glandular tissue. Adipomastia, by contrast, is mainly an excess of fat, without this glandular proliferation. The surgical technique is chosen according to the proportion of fat and gland.

The result of gynaecomastia surgery is usually long-lasting when the cause of gynaecomastia has been treated or removed and weight stays stable. Significant weight gain, certain medicines, hormones, anabolic steroids or other substances may increase the risk of recurrence.
