The procedure

Otoplasty moves the ear back by folding and fixing its cartilage with deep stitches. The main scar sits in the fold behind the ear.

The approach depends on what makes the ear stick out: insufficient cartilage folding, an overdeveloped or overly projecting concha (the hollow of the ear), or too wide an angle between the ear and the skull. Otoplasty gives a lasting correction of prominent ears by reshaping and repositioning the cartilage, for ears that stand out less and look natural. Some residual asymmetry may persist, and ageing, growth in children, knocks and individual tissue reactions may alter the result over time.

Before otoplasty, Dr Manon Belhoste carefully examines both ears to assess the cartilage abnormalities, the projection, any existing asymmetries and the corrections being considered. No medicine containing aspirin should be taken in the 10 days before the procedure, unless a doctor advises otherwise, and an anticoagulant or antiplatelet drug is never stopped on its own. All treatments, allergies, illnesses, clotting disorders and previous surgery must be reported. Stopping smoking and all nicotine is recommended before and after the procedure. The head and hair should be washed thoroughly the day before or on the morning of the procedure, and a hairstyle that fully exposes the ears is advisable.

Swelling, bruising, sensitivity, a feeling of tightness and numbness are usual at first, and the ears may look very flattened, swollen or slightly asymmetrical until the swelling goes down. Sleeping directly on the ears should be avoided early in healing, as should knocks, contact sports and friction for a few weeks.

Otoplasty is possible in children once ear growth is sufficiently advanced and the need is genuine. Otoplasty is commonly considered from around 7 years of age. Otoplasty can also be carried out in adolescence or adulthood.
